Analysis & Patient Relations. Obtain names of newly admitted patients going to 4th or 5th floor via ER Track Board and/or Nursing Bed Manager, and upon request by Nursing/ Hospitalists. Interview newly admitted patient or patient's family to obtain the most accurate and current list of home medications the patient is taking. It is critical that the Historians create a positive and safe environment to gather best med information. Contact patient's family, nursing facility (MAG or MAR obtainment), home pharmacy, and/or physician office for medication clarifications as necessary. Input best and most accurate obtained home medication list into EPIC. Obtain and input into Epic information on what primary home pharmacy they utilize and any secondary pharmacies they may use. Obtain and input into Epic a detailed description of drug allergies and what reaction occurred due to that allergy (i.e. allergy = morphine which causes reaction = shortness of breath).
