Responsibilities
Maintains a working knowledge of applicable PACE Federal, State, and local laws and regulations, the Compliance Accountability Program, Code of Ethics, as well as other policies and procedures in order to assure adherence in a manner that reflects honest, ethical, and professional behavior.
• Obtains and evaluates nutritional and medical history, laboratory data, test results, current medications, current diet orders and nutritional regimen to determine participant's estimated nutritional needs.
• Recommends and implements nutrition interventions that assist in maintaining or improving nutritional-related medical diagnoses.
• Completes nutrition assessments for participants per regulatory guidelines and when needed to assist in developing participant's plan of care and service delivery requests.
• Instructs participants, family members, staff and contracted providers/facilities on diet, food safety and sanitation.
• Documents pertinent participant information in the electronic medical record (EMR) and the participant plan of care.
• Attends Interdisciplinary Team meetings daily and team activities when scheduled.
• Performs other duties as assigned.
