Licensed Practical Nurse (LPN) - Facility & Home Health

We are seeking dedicated Licensed Practical Nurses (LPNs) to join our agency team! As an LPN with us, you'll have the flexibility to work in both facility settings (nursing homes, assisted living, rehab centers, and hospitals) and home health care assignments, ensuring patients receive quality care wherever they are.

Responsibilities

• Provide nursing care under the supervision of RNs and physicians
• Administer medications, injections, and treatments as prescribed
• Monitor patient conditions, record vital signs, and report changes promptly
• Support residents and patients with daily care needs while maintaining dignity and respect
• Assist with wound care, medication management, and chronic condition monitoring in the home
• Document care accurately and follow agency, state, and facility policies

• Current LPN license (state of practice)
• CPR/BLS certification
• Strong clinical and organizational skills
• Adaptability to different care environments
