Floyd Valley Healthcare in Le Mars, IA has a full-time Social Worker position available.

Social Worker Responsibilities:
- To assist patients of Floyd Valley Healthcare and Family Medicine Clinics to resolve social or emotional problems that are or are expected to be an impediment to the effective treatment of the patient's medical or mental conditions or rate of recovery.
- To assist patients in maintaining or improving their ability to manage their everyday physical, mental and psychosocial needs.
- Develop patient centered discharge plans in collaboration with Case Manager, physicians and nursing.
- Minimum of a Bachelor's degree from a school of social work accredited by the Council on Social Work Education. Master's is preferred.
- One year of experience.
- Mandatory Reporter training certificate a plus; certification is required within six months of employment.
- BLS certification within six months of employment AND current certification maintained throughout employment.
- Licensed by the State of Iowa for social work.
- Must possess knowledge of Medicare and Medicaid guidelines.
- Good oral and written communication skills and strong organizational and leadership qualities.
