MAIN FUNCTION: The team lead establishes, administers, and directs the Assertive Community Treatment (ACT) team, a self-contained clinical team which assumes responsibility for directly providing needed treatment, rehabilitation, and support services to identified clients with severe and persistent mental illnesses; supervises and evaluations the multidisciplinary team in conjunction with appropriate psychiatric support to ensure service excellence and courteous, helpful, and respectful services to program clients; and functions as a practicing clinician on the team.

DUTIES/RESPONSIBILITIES:

  • Provides all services in accordance with the policies and procedures of Heartland Counseling, applicable professional code of ethics, and all payors, licensing, and governmental rules.
  • Provide case management for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team; assume primary responsibility for developing, writing, implementing, and evaluating the overall treatment goals and plans; acts as a liaison with community agencies and families; make referrals to services and related activities to assist the client with identified needs.
  • Provide individual supportive therapy and management, advocating for client's rights and preferences. Provide direct clinical services to clients on an individual, group, and family basis in the office and the community to teach behavioral symptom-management techniques and develop a trusting relationship with clients.
  • Conduct comprehensive assessment of psychiatric history, mental status, and diagnosis; physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living; and family structure and relationships.
  • Perform shift management in coordination with other ACT shift managers according to established policies and procedures.
  • Provide on-call crisis intervention covering nighttime hours and serve as a backup to evening and weekend staff, including direct assessment and treatment of the client's urgent or crisis symptoms.
  • Document client progress to maintain a permanent record of client activity according to established methods and procedures.
  • Provide ongoing assessment of clients' mental illness symptoms and client's response to treatment; make appropriate changes to treatment plans to ensure immediate and appropriate interventions are provided.
  • Provide symptom education to clients and their families.
  • Provide individual vocational-supportive counseling to enable clients to identify vocational strengths and problems, establish vocational or career goals and plans to reach them, and recognize and target symptoms of mental illnesses that interfere with work; plan and provide work-related supportive services such as assistance with grooming and personal hygiene, securing of appropriate clothing, wake-up calls, and transportation; conduct on-the-job performance assessments and evaluations, regular work review sessions with clients and their employers, on-the-job support; coordinate with vocational rehabilitation and other employment services; provide benefits counseling (i.e. SSI).
  • Participate in treatment, rehabilitation, and support services, including psychiatric treatment, substance use disorder services, employment, activities of daily living skills, social and interpersonal relationships, and advocacy, problem solving, direct assistance, and training.
  • Direct the day-to-day operations of the team, including scheduling to assure appropriate coverage; lead the daily organizational staff meetings and treatment planning meetings; continuously evaluate the status of clients and do appropriate planning and coordination of services.
  • Direct and coordinate the client admission process and treatment, rehabilitation, and support services of the program in coordination with the psychiatric provider. Schedules admission interviews, develops/coordinates initial assessment and treatment plan, assigns staff, and provides clinical supervision of the development of comprehensive assessment and treatment plans.
  • Direct and coordinate for each client the comprehensive assessment of psychiatric history, mental status, and diagnosis; physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living; and family structure and relationships.
  • Participates in staff recruitment and hiring processes; develop and implement staff orientation and training.
  • Assist in managing the program budget.
  • Supervise medical records management assuring maintenance of the medical record in compliance with agency policies, Medicaid, and other third-party payment requirements; train staff on medical record requirements; regularly review client assignments, treatment plans, and progress notes written by staff; and supervise individual staff for medical records mastery.
  • Conduct and document quality-assurance activities and reviews of use of program services.
  • Review and maintain program policies and procedures and revise as necessary.
  • Initiate and maintain relationships, in coordination with other staff, with law-enforcement, and other human-service agencies, and with informal community resources.
  • Perform on-call duty for crisis intervention and for staff supervision and consultation.
  • Performs other related duties as assigned.
REQUIRED SKILLS/ABILITIES:

  • Possess a valid driver's license and an insurable driving record.
EDUCATION & EXPERIENCE:
  • Independent clinical license in the state of Nebraska (LIMHP, LICSW, LIMFT).
  • Master's degree in social work, psychology, counseling, marriage and family therapy, art therapy, or a related field.
  • Supervisory and program management experience is desirable; experience with new-program start-up preferred. This person in this position must have a clear understanding of the characteristics and problems of adults with severe and persistent mental illnesses and be knowledgeable of the ACT model.
