Pre-admission Referrals/ Assessments. Acknowledges all referrals to IRF and communicates to team. Is knowledgeable of post-acute levels of care and their admission criteria, i.e. IRF, SNF, LTACH. Assures IRF admission criteria are met in accordance to CMS regulations. Conducts pre-admission assessment and completes the required documentation. Communicates the pre-admission assessment to rehab physician for further review and admission decision. Meets referred patient/family to review the IRF program/services and identify goals and discharge plan prior to IRF admission. Marketing/ Referral Relationship Development/ Volume Growth. Fosters excellent communication between referral sources, physicians, insurance companies, and the interdisciplinary IRF team. Participates in performance improvement opportunities related to IRF referral process. Assures monthly/quarterly referral tracking data is complete and accurate. Care Coordination.
